PLAYER REGESTRATION INFORMATION
Harrow St Mary’s Youth FC is a non-profit organisation run entirely by unpaid volunteers.            Signing on fees covers all playing and training costs, also including:
                 • Training and match equipment                                                                                                                                         (team kits, balls, goals, nets corner flags, bibs, cones, first aid kits ect)                                                                                           • Public liability insurance,                                                                                                                                                                             • Cost of hiring pitches,                                                                                                                                                                                     • Club and Player registration with the relevant League and Middlesex FA,                                                                  • Coaching, first aid training and child welfare  courses for managers/coaches,                                                   • Club events such as Presentation Day and Firework Night.

                                                                                         Season Fees £175.00.

   Player reference (abbreviate if need be, for example U15b J Smith instead of... John Smith Under15 )   

  Cash: Must be in a sealed envelope and clearing labelled with Players name and Team.  

Card payments can by made at the Clubhouse.     
            

 NOTE: Players are not eligible to participate in training or matches unless payment has been received.                                                                                                                                                                                                                      Fees are non-refundable except in exceptional circumstances.
 

Registration checklist Please check and ensure the following has been supplied/completed:

• This form has been read and all areas completed

 • Both Player’s and parent/guardian signatures and information must be present (players U18 will not be registered without this )

• Full payment has been made/provided

• A passport style photo has been supplied (as a jpeg)

 • You have read and understand the Club’s code of conduct, social media policy and the FA Respect literature available on the club website: www.harrowstmarysyouthfc.com 

• If you are new to the club supply a proof of age such as passport or birth certificate,                                  (photocopy - no originals accepted)
 

All information should be collated and primarily handed to the team manager/representative or if otherwise arranged, a member of Harrow St Mary’s committee.
